While our business office will do its best to help you obtain maximum benefits, due to the many changes in healthcare insurance, it is important for you to know your individual plan. We will secure any information regarding co-payments, coinsurance, and/or deductible amounts due from the patient. These fees will be your responsibility at the time of your procedure. We will make every attempt to notify you of the amount prior to surgery. If we have been unsuccessful in reaching you, you will be given the amount at check-in on the date of your procedure.
We accept most types of insurance plans, such as Blue Cross Blue Shield, Health Alliance, Private Healthcare Systems (PHCS), John Deere, Medicare, as well as many others. Please bring your insurance card with you on the day of your procedure so a copy may be made.
If you have no insurance coverage, please come prepared to pay the Endoscopy Center's facility fee, in full, the day of your surgery. Prior financial arrangements can be made by contacting our business office.
You will receive two bills. The physician and the facility (Digestive Disease ENDOSCOPY CENTER).
The amount due is calculated on base fees and can be adjusted dependent upon the final outcome of your procedure. You will be billed for all remaining balances.
In addition to our facility bill you may receive separate bills from:
- Pathology
- Radiology
- Laboratory
Forms of payment accepted are cash, check, Visa, MasterCard or Discover.
